First step would be to create a documenttype ( ex. ContactInfo ) In the documenttype you create a new tab ( ex Info ) And You create properties on the tab ( ex name, phone, ... ) Now you can create a form based on the doctype. Select the template where you want to add the autoform ( be sure you have the asp_form on it, or on its parent ) Choose insert macro, autoform Choose the documenttype ( ContactInfo ), the tab ( Info ) and where to store the content submitted with the form.

Not Really. Here is a quick how-to I sent to someone the other day... 1. Create a Document Type that contains tabs and the fields you want to collect in the form. 2. Create a Template for the Document Type that you want to include the form on. (you can create a Document Type if you want to make it simple) 3. Add the Macro (there are a few options) to the template. Specify the Document Type set in Step 1, and the Tabs from Step 1. Continue setting the properties for the macro. If you have questions, feel free to ask. 4. Add a node in the content section to display the form on. If you created a Document Type for the form itself, be sure that you have allowed that Document Type as a child type on the Document Type of the node that you want to add the form under. (Wow, that is confusing.) If you just created the template, be sure that the template is associated with the Document Type that you are using. Optionally, you can use the alternate template method. 5. Test the form by visiting the published node that the form has been setup on. I will try to make a How-To soon. Case

Yes, you can use AutoForm, AutoFormPlus or Doc2Form Beta

For email sending, you will have to use the latter two.
