Does anyone know if it's possible to use <?UMBRACO_TEMPLATE_LOAD_CHILD/> from within an XSLT macro?

It isn't but I think you could use the umbraco.library:RenderTemplate() xslt extension instead. You just pass the parameters of you current page, and the template you need to render...
