umbracoUseDirectoryUrls issues Options

We switched umbracoUseDirectoryUrls to true and made the necessary wildcard handler change in IIS. Now, certain media files are missing. It almost seems like something rolled back. Any idea why? The site only gets about 2,500 visitors and 15,000 page views per day.
